Core Standards

The 2026 FHIR Landscape

Four standards define interoperability in 2026. ChartScribe implements all of them.

FHIR Release 4.0.1 (R4)

Implemented

The current production standard used by almost all certified EHRs (Epic, Cerner, EMIS, SystmOne, etc.). Normative core resources + REST API. Recommended base for all new development in regulated markets (ONC, NHS, EU EHDS).

ChartScribe Use

ChartScribe exports every clinical note as a FHIR R4 Composition wrapped in a document Bundle. Uses Patient, Encounter, Observation, Condition, CarePlan, and MedicationRequest resources.

US Core Implementation Guide (v6–v9)

Implemented

The most important profile for real-world use. Defines minimum constraints on FHIR resources so data is consistent across systems. Aligns with USCDI v3/v4 (the minimum dataset that must be exchangeable).

ChartScribe Use

All ChartScribe FHIR exports apply US Core profiles: us-core-patient, us-core-practitioner, us-core-encounter, us-core-observation, us-core-condition, us-core-careplan, us-core-medicationrequest.

SMART on FHIR (v2.2)

Implemented

The standard way apps launch inside or alongside EHRs. Handles authentication, patient context, and scopes. v2.2 is the current production version with enhanced security and granular scope control.

ChartScribe Use

Patients launch trusted health apps (Apple Health, MyFitnessPal) from .care. Clinicians open patient-context views from Epic/Cerner. Supports patient, EHR-context, and carer launch types.

Bulk Data Access (Flat FHIR / $export)

Enterprise Roadmap

Allows exporting large populations (e.g., all patients in a practice) in NDJSON format. Essential for Enterprise tier reporting and population health analytics. Bulk Data v2 is the current production version.

ChartScribe Use

Enterprise customers can export all notes, patients, and encounters for a given period via FHIR Bulk Data $export. Returns NDJSON files for ingestion into analytics platforms.

Key 2026 Transitions

R5 / R6 Status

Bulk Data Version

v2 — current production standard

NHS / HSE Position

Heavily pushing FHIR R4 + US Core + SMART

ChartScribe Specific

Practical Relevance to ChartScribe

Doctor Side (.io)

  • Export generated notes as FHIR Composition + Bundle → push to any modern EHR
  • Pull patient data (medications, allergies, history) to pre-fill notes
  • Support Bulk $export for practice managers (Enterprise tier)
  • C-CDA fallback for legacy systems without FHIR support

Patient Side (.care)

  • Patients launch third-party apps (Apple Health, fitness trackers) via SMART on FHIR
  • Clinicians launch .care views with full patient context from their EHR
  • Secure OAuth2 + patient consent on every launch
  • Family/carer access with delegated consent and relationship tokens

Competitive Advantage

  • "FHIR R4 + US Core + SMART Ready" out of the box — strong marketing claim
  • C-CDA alongside FHIR covers both modern and legacy systems
  • NHS DTAC aligned — pre-qualified for UK procurement frameworks
  • Enterprise Bulk Data for population health analytics and reporting
Roadmap

Implementation Priorities

  • FHIR R4 Composition export + US Core profiles (already implemented)
  • Basic SMART launch for .care — patient and clinician context (already implemented)
  • C-CDA R2.1 generation for legacy EHR compatibility (already implemented)
